doug mcelway has more from washington. >> reporter: the trouble is none of these up to ten deceased patients at the lewis johnson va medicalr were diabetic. insulin can be fatal to those without diabetes. investigators believe that was the intention, to kill them. this all came to light when investigators asked the daughter of 82-year-old army veteran felix mcdermott if they could exhume his body. she agreed and the autopsy confirmed mcdermott had been given the insulin shot in his abdomen, effectively killing him and potentially the ten other patients. >> there's a complete system failure here that allowed this many people to be killed without any accountability. mr. mcdermott did suffer from dementia. the ones that i have talked to or that i have seen seems that's one of the common traits, is that these people were defenseless. >> west virginia senator joe manchin said he was told as early as july of 2018 of mcdermott's death and that deficiencies had been corrected but there was no mention at that time to him of potential murder. >> we did not know that there was a homicide that was connected with this activity way back
