health plans are introducing new plans that have alternative provider networks. you may have heard the term narrow network lands -- plans. a number of plans will introduce alternative plans with narrow network that can help drive down costs because they're able to negotiate lower rates with providers if those providers want to be in the narrow network. i think we have seen that has been the case in the rates released by california last week. they have been able to support lower premiums than what we otherwise would have expected. so our study did not want to look at what effect that might have an effect might be a 10 or 15% lower rate which otherwise might be affected. that kind of counters for the people who opt for that type of land, the trend increase i just mentioned. we see increases on average between 15 and 60%. new jersey, on the other hand, we see the average will be somewhere between -25% and zero percent. that is for the sample plans we looked at. of those pieces, a good amount of that is due to the fact that people will need to upgrade their benefits

one is nothing prevents a group health plan or insurer from discriminating in favor of an individual with an adverse health care condition. that is called benign discrimination, so you can do something that benefits somebody with a health care condition or you can have a health wellness program that needs regulatory requirements then laid out in the 2006 regulations. so along comes the affordable care act three years later and congress wrestles with this whole issue. they basically codified the 2006 wellness rules and then increase the allowable incentives that are available for a program under these regulations from 20% to 30% and then gave the secretary of hhs discretion to have incentives all the way up to 50%. >> paul, could i ask you 20% to 30% of what? >> said the 20%, 30% of the premium contribution by the employee, the maximum amount would be based for employees only coverage unless the wellness program is also available to the employee's dependence on them that would be 20% or now 30% of the cost for family coverage paid by the employees, said the employee's contribution that

as a result, a lot of companies are "carving out" spouses from company health plans. it's one of the trade-offs, negotiated to win passage of the affordable care act: to cover kids longer, members of congress made coverage for spouses optional, and while not naming them specifically, may have been thinking of women. "they tend to use healthcare more, expecially if they're not working. they're free to go to the doctor more and often have more reason to go see the doctor." the changes take effect next year, the same time a network of health exchanges for people to buy coverage opens for business. "employers are doing everything they can to transfer the cost of healthcare to employees. the cost shift is going to be significant this time around." some companies offer surcharges for spousal coverage, but make them so high or reimbursement rates so low they in effect discourage coverage. between now and the end of the year, a lot of couples will likely have "the talk" about how much health coverage they're willing to pay for their non-covered spouse.
