medicare patients pay more. i understand these proposals, while the concern we were put into the budget is a part of a so-called big bold balanced-budget deficit reduction plan, one that calls for sire advice -- shared sacrifice among retired americans. let me ask this question. completely seems to be around here is much different is that you can cherry pick those medicare reforms here and one there such as low hanging fruit as a way to offset or pay for sgr. my republican colleagues have been talking about doing this. for thathat it is hard to actually occur because as gr needs to be fixed. there's not a documented access problem to our program. askeems unconscionable to household incomes averaging $23,000 a year to pay more in order to increase payments to doctors. my question is this. does the administration support cherry picking structural reforms which would benefit for medicaret beneficiaries, or are there was reforms solely intended as a part of a substantial deficit reduction package with shared sacrifi

medicare, this is a figure of medicare spending for different components. 11% is outpatient prescription drugs, then there are drugs administered in doctors' offices, drugged administered in hospitals and medicaid. if you put that all together, we're talking about at least $100 billion a year in sales for these products that the government develops with one hand and then buys from the companies with the other. so let's do our thought experiment. what would the industry look like had the government not sewer screened. without -- intervened. without question we would have fewer drugs, lower profits for the companies, less public trust in pharmaceuticals without the fda and certainly nowhere near the problem fears of -- frontiers of genomic medicine that we are facing today. let me briefly describe the other industries i look at. hospitals, a quote from an ethicist describing hospitals as pawns of medicare. that maybe overstating it a bit, but the point is that medicare's indispensable to the american hospital industry. hospital construction in constant dollars going up very steadily over t

the first year i was a medicare trustee. in 2009 the anticipation was the actual aerials, projection was that medicare would begin to be insolvent, not that they wouldn't have any money that they would have about 70 cents on the dollar by 2070. so in 2009 it was a 2017 cliff. the passage of the aca added years to that solvency, according to the actuary who looked at the law and the impact over time. and subsequent budgets have also added years, so we are now -- the 2015 budget, according to the actuarial projection, would add an additional five years to the solvency of the medicare trust fund. so during this administration i was a significant solvency years have been added. >> are the republican trustees of? >> yes. >> do they signed the document? >> yes, they do. >> and did a signing? >> yes. >> my point is, an example much like the one that secretary lew present about deficits in his appearance before the committee recently, and it's frequently underreported in terms of the good news because the emphasis remains on the con

second, i offer the medicare s.g.r. and beneficiary act assess improvement act of 2015 in order to eliminate the ongoing threat to our seniors and the providers who serve them. under this legislation which reflects the bipartisan, bicameral legislation that senator catch and senator baucus offered last month, physicians would receive annual payment increases of .5% for five years. the following five years, physicians would not receive automatic increases but rather would be eligible for payment increases based on performance. medicare would transition to a new focus on greater quality, value and accountability. this legislation would strengthen medicare physician payments in a number of ways. it would reward the quality of care. it would improve payment accuracy, expand the coordination of care for patients with chronic care needs and encourage participation in alternative models of payment. and the bill addresses other critical medicare and medicaid issues. they are known as health care extenders. and with these extend
