zeke emanuel. dr. emanuel is a physician who served as a health policy adviser in the obama administration. he's now the vice provost for global initiatives at the university of pennsylvania. dr. emanuel is also an nbc news and msnbc senior medical contributor. zeke, good to see you again today. let's start things off with you giving us a sense of where we are in this crisis. your vital signs for this moment in our national life. >> nice to be here with you, ali. i have three vital signs that really stand out for me tonight. first, we're not going to get out of this crisis without testing, and right now we're not doing nearly enough testing compared to other countries when it comes to how much of our population can get a coronavirus test. you can see in this slide we're behind. second, in places where we're able to test a lot, like the navy's "uss theodore roosevelt," they found that 17% of the crew had gotten the virus and an amazing 60% of the positives had no symptoms at all. well, they are sailors.

zeke emanuel, he served in the clinton and obama administrations.s now the vice provost at the global initiatives for university of pennsylvania and an msnbc senior medical contributor. zeke, the rockefeller foundation plan is the most ambitious i've seen. how do you get to that level of testing? >> well, they don't suggest that it would happen overnight. they have suggested that you would ramp up over time that you would have an emergency next eight weeks trying to increase the testing capacity, mostly by utilizing labs and universities and other places and optimizing labs, and that they would create an accelerator so that we could figure out better ways and faster ways to test over the next six months. so it's really looking at 30 million a week over the -- to get to the next six months. and importantly, they sort of don't just pull this number out of the hat. we looked at what south korea is doing. and if you tie very, very strategic contact tracing with testing, you can probably get away with about 2.5 million tests per day, multiply that by sev

zeke emanuel and you guys are out there. as you know, we're in a new era broadcasting from our homes and every now and then we get some technic technical gremlins into the system. thanks for your patience. zeke emanuel, i want to ask you about this report that came out that suggests that there was a test that was conducted at the university of chicago with an existing drug and 133 people were tested and all but two have recovered from serious coronavirus. two people perished. what do you know about this test and what should we make of it? >> so remdesivir is an anti viral agent that works to disrupt the reproduction of the virus once it's in the lung cells. it was developed, i don't know, about 18, 17, 18 years ago by gilead. never found a home for use. it's been tried in several different conditions. and there is some early data that it's promising and working. now, you know, the big problem in all these trials where you don't have a randomized trial setting and you don't randomize patients is what's called patient selectio

emanuel, can you talk about what it must be like for dr. fauci to function, dr. birx to function on this task force where as you just said, you didn't make up the number 18 months, it's based on science and memos and the possession very much available to the president and the other political leaders, what do you think that co-existence looks like? >> i don't know for sure but i think, you know, one minute the president sounds contrite and he sounds like he really understands the magnitude of the problem, 100,000 to 250,000 deaths, wow, got to physically distance, we takes the public health measures seriously and then he flips back into his usual mode and usual intuition, which is, he said, let's be a cheerleader regardless of the data. biology doesn't listen to press briefings or pressers. biology has a truth to it and you'll be hurt by that truth if you don't pay attention to it and don't try to learn from it. i'm a cancer doctor and this happens all the time. someone paints a rosy picture for the patient and then the tumor pops back. i think the next serious pr

emanuel said, 250 might have been wishful thinking. those decisions could be made today. you should be picking what happened the last two months. for people to say stop harping on the past, the reason you have to harp on the past, is that this will happen again. there are steps that you could write down in a book. they were written down in a book, by the way, of what you have to do if a pandemic is coming. how do you have to mobilize your industry? how do you have to move into lockdown? how do you have to mobilize the federal government in the stuff is known and now it's even more known and hopefully people now put this into practice going forward. >> and, yeah, we do have to continue keeping the pressure on. you know, i've been calling for -- i'll just say it, i've been calling friends in the white house for a couple weeks now begging them to do two weeks ago what they did yesterday. begging them to be more serious. as you know, when i came on this show i irritated a lot of people by encouraging the president, agreeing that we're all on the same team when he held press c

emanuel, let's talk about a couple of things that came out overnight. "the washington post" reported the interview with the cdc director, dr. redfield, warning a second wave next went wither coinciding with seasonal flu could be more difficult for hospitals and more difficult for the whoefl medic the whoele medical community. what say you? >> yes, i think he's recognizing, first of all, we could have a second wave of covid-19. he's arguing a third wave, if we have opening up in georgia, texas and tennessee, you might see increases early as july and the fall would be maybe a third wave. and i think what is worrying him is that we'll have two very serious viral infections out in the fall. we know we get typically a very big increase in hospitalizations from flu, especially a bad flu season, adding coronavirus to that with obviously really taxing the system and obviously might dramatically increase, especially if people get coinfected with both agents. it might dramatically increase our death rate. so are his worries. that it's coming now suggests this rus

"without federal support many cities will be forced to lay off employees and make cuts that will hurt public safety." what's the worst case scenario, that the mayor of chicago, mayor of new york, mimelyi, l.a., so many mayors around the country are facing right now if they don't get a lot of federal financial support and don't get it soon? >> well, wolf, they're going to make -- they're going to be faced and the publics are going to be faced with horrible choices between public safety, public health and all the basic services mayors are trying to keep going in their cities. just think about also all the sanitation you're going to need and the capacity. look, the economies in their cities

let's bring in zeke emanuel here.i hope you don't mind me telling people about tfr conversations we've had with your brother and you. i remember ari, you, and me talking at the end of february. i've got to say, your brother, he gets so many things wrong, but i remember him calling me in the middle of the night, flying back from europe, when the stock market was at 29,000, saying, "we're heading into a recession." i was like, "come on." i remember at the end of february, you telling me that you were concerned. end of february. i want everybody to stop for a second. a couple of months ago, on the phone with zeke emanuel, ari is on the phone, too, and zeke, you said at the end of february, "what we really need to be concerned about, if you look at the 1918 pandemic, yes, it was bad in the spring. it was worse in the fall. we've got to ramp up now, not just for what's about to hit us. we've got to ramp up for what may be even worse in the fall." you said that in february. the president yesterday denying it. yesterday, tha

emanuel, let's talk about this prognosis, this forecast. it seems as though the president has now accepted what his science advisers are saying. they're no longer talking about easter reopenings. do you think april 30th is enough? we look at these forecasts from the university of washington they seem to be heeding and they're going right through may. >> yeah, i think we went from a 15-day plan that vice president trump was waving around and now we have an additional 30-day plan and you saw dr. fauci say the next two weeks are going to be tough. i think we're still underplaying what is necessary. every time i look at the data, and that includes the university of washington data, but it includes lots of other data from around the world and lots of other projections, it's really eight or ten weeks of time. it will take you four weeks to get to the top that tony fauci has been saying. that is a lagging indicator. we have to see new cases go to zero. we have to see a plateau in the percentage of people who are positive, and then we can't ease of

zeke emanuel. first of all, your reaction to taking back the money and the house democrats appropriations committee, as you pointed out, he doesn't have the right to impound this money without going to congress. we're talking about state department appropriated funds that were supposed to be sent hundreds of millions of dollars in previous years to the world health organization. without judging the world health organization at this moment, pluses and minuses, what they did, what they didn't do, the criticism around the world is this is the worst time for the u.s. to be sending this signal globally that we are not cooperating and supporting a u.n. organization that does a lot of other things, polio and other diseases as well. >> so let me make thee points. first, let me be transparent, i've been a special adviser to the secretary-general there for a few months now at the w.h.o., and i think it is a very important organization. it's not without its flaws that are well known to the organization itself

zeke emanuel, an expert on public health.e chair of the department of medical ethics and health policy at the university of pennsylvania. he's also a cohost of the podcast, "making the call." make the call here. should these governors be doing this? >> well, first of all, you know, let's talk about the protests. we've had protests from a few hundred people in states that have millions, tens of millions of people. we should remember, this is a small minority. the second thing is, these governors are opening up their state to business and you just highlighted, you know, vsalons, gyms, probably not the place you would start if you were doing this on a public health basis. you would think of places where you can have social distancing. that might be -- you wouldn't have someone interacting with so many people actually, literally, touching them and coming in close contact with them. so, they don't seem to be well-informed by public health measures. and the last point i want to make clear is it may be that some people in these stat

zeke emanuel. what is the difference in the type of modeling new york is experiencing with several days of decreased hospitalizations, decreased intubations, and decreased mortality? >> it's a positive sign, and i think as the governor correctly pointed out, it probably does suggest that the physical distancing started in new york is beginning to have its impact on the number of people infected, the number of people who get serious infections, and that's a good thing. it first of all shows that public health can work. it shows that physical distancing can have a positive impact on the disease trajectory, and i think equally, as the governor made clear, that doesn't mean we're out of the woods by any stretch of the imagination. we have to keep this up. i think, you know, most of the public health community thinks that, again, assuming the physical distancing stays in place -- people wash their hands and continue and wear masks out in public, that it won't be a prolonged plateau, but it will begin to
