here's the latest in fairfax, mark segraves, news 4. >> thanks, mark. >>> from destruction to disaster investigators say that's what happened to a train operator during an amtrak derailment in philadelphia. eight people died in the crash last year. federal investigators testified in a hearing in washington. they concluded that engineer brandon bastion was so focused on radio traffic about a rock that hit another train. he lost track of where he was and accelerated as the train approached a steep curve. now there is another factor that investigators pinpointed, as hour. >>> right now we are learning more details about back-to-back murders that happened just a few blocks apart. those case are two of five shootings that we saw across the district in just a matter of hours yesterday. news 4's pat collins has the latest in southeast. >> a woman gets out of a car on 49th street southeast and as she walks to the front door of her house she's gunned down and falls dead to the ground. neighbors say they heard about six shots. the victim identified as tracy cooper, a woman in her 40s. police bel

mark segraves joins us live from the scene with details. mark? >> reporter: yeah. this sign right here, this is the only thing preventing someone from using this pathway to cut across these tracks from the shopping center into one of the neighborhoods, and as you said, neighbors say they constantly see teenagers and even children walking and playing along these dangerous railroad tracks. >> it's super dangerous. i mean, sometimes i tell them to get off and not many cops circle around here as much, so it's pretty dangerous, yeah. >> it was about 9:30 last night as three teenagers were walking along these train tracks in fairfax, virginia, one of them >> erica dunn died from her injuries at a local hospital. she'd graduated from nearby lake braddock high school last year. according to the federal railroad administration since 2014 there have been 29 people killed while walking along train tracks in virginia. in 2015, a 15-year-old girl was hit and killed by a train not far from where they were that night. neighbors know how dangerous it can be. >> nowadays, at some l

. >> mark segraves reporting from burke, virginia. thank you, mark. >>> according to the news 4 i-team dozens have been killed. dunn is the 29th death since 2014. more than 100 trespassers were injure order killed by trains between september 2011 and september 2014. >> officials at amtrak say they're reviewing the findings from federal investigators on what probably caused that deadly derailment in philadelphia. eight people were killed as a speeding train rounded a curve near philadelphia last year. the national transportation safety board says human error is to blame. the train's engineer was distracted, we are told, by radio traffic. investigators also point a finger at at track for not installing positive train control and that is technology which automatically slows trains down that are going over the speed limit. i'm very concerned that we're going to be back in this room again, hearing investigators detail how technologyt 45 years could have prevented yet another fatal accident. brandon pastian, the engineer operating the train

mark segraves, news4. >>> immediately after the shooting, the secret service and d.c. police focused on a parked car possibly belonging to the gunman. >> jackie bensen is along 15th jackie? >> reporter: jim, a d.c. police mobile crime unit just rolled up to this scene. what we have been watching here are investigators going up to that vehicle, the white chevy malibu you can see over there with the trunk and the hood up with pennsylvania license plates, they have been taking out items inside that vehicle and putting them into brown paper evidence bags. we saw a white plastic bag that appeared to have something in it. i can also tell you we have noticed that there are a group of three women here who are dressed as tourists. those women have each been questioned separately and extensively by d.c. police and have been here this entire time and appear to have been asked to wait here to talk to investigators. there is a possibility that these women, these visitors, were witness to something that happened down here prior to when this man walked onto the grounds of the ellipse

mark segraves is near the scene now. have they figured out why this happened yet? >> reporter: hey, jim. i can tell you federal inspectors as well as d.c. inspectors have been on the scene since yesterday. they say it is say what may have caused this derailment. you can take a look down behind me and you can see work is still under way. as to that video that we were showing you earlier, very dramatic video of the derailment and all of the cars piling up. during that video, one of the angles shows sparks flying from beneath one of the tankered cars. i actually asked one of the csx officials here and a federal inspector about that video. they say it is too early to read anything into it. it will be part of the investigation. they are looking at that video and they are inspecting these tracks. they're interviewing the crew of the train. it could be up to a year before they can say what the cause of this derailment was. >> these tracks are inspected visually several times a week. they are inspected between three and 12 times a year by a system that uses ultrasound to e

mark segraves is live with what all this is going to mean for people. mark? >> reporter: good afternoon. whether you ride metro or not, it is going to impact you. tomorrow metro's new general manager is expected to roll out a complete top to bottom rehab of the metro rail system. much of that system dates the result we'll see much of the underground system will be played out over the past years. unreliable service and fatalities. the general manager will announce a plan that will include the closure of some portions of the system. larger portions of the system could be reduced to single tracking, which will also create serious delays. the chairman says while this is going to be painful for the entire region, the alternative is much worse. >> you know, we don't have to do any of this. we can just do what we're doing and it can catch fire in four days and people's safety will be unreliable. >> reporter: we expect to get details tomorrow morning when the gm has his press briefing at 11:00 a.m. back to you. >> what are other local leaders saying about the prospec

mark segraves explains. >> reporter: when he moved to d.c., he did what most people do when they movend his boyfriend went to the dmv to get new driver's licenses. they also registered to vote using the same form. but when they checked their voter registration status in advance of the june primary, they found out they weren't registered at all. >> it was a little scary. before i moved to d.c., i never missed an election. i know my parents would have killed me if i did. >> reporter: while the board of elections and the dmv can't say how many times something like this may have happened, but prior to a few months ago the motor voter process in d.c. was all done on paper. the completed forms went where is the registrations were entered by hand. while it is possible their forms were lost or there was an input error, there is another problem. residents sometimes forget they have to not only check the box to register to vote. they also need to sign the form. board of elections officials say as of february this year they're using a new way to transmit the applications to avoid just these type
